I recently used www.pingo.com to call Nepal (Kathmandu). I think this is the best one I got right now...It is NOT a phone card. Excellent voice. Call got through in the first attempt.
Here is the min and rate I was charged (Copy pasted from my history of calls). No connection fee.
If anybody is familiar with onesuite.com. It is exactly like that, just cheaper to call Nepal.

Using for last two months. Voice quality is good. But sometimes, I had to call two-three times to get connected. On number of occassions, line breaks in the middle of conversation. No connection fee.
$5 = 45 mins. I think it's better than other phone cards.
